Enclosed with this letter is a completed Change of Information Form that outlines my resulting change in status and contact information and a copy of [supporting document, such as Order in Council, if applicable]. I also include information concerning my files, client property and trust accounts, below [if applicable].

Trust account(s): [include statement that you had no trust account(s), that you are working to return trust funds or that trust account(s) are at a zero balance and have been closed]

Open files: [indicate if any files were still active at time of practice transfer or closure and contact information for lawyer who took over active files]

Closed files: [indicate whether you will retain closed files and for how long, or provide contact information for lawyer who took over storage or closed files]

Original client documents: [indicate if you held any original client documents and whether you will retain these, have forwarded them to clients or provide contact information for paralegal/lawyer who took over storage of these]
